We are so glad you are here. Aevia Integrative Health is a small, nurse-led practice dedicated to supporting women through personalized hormone and wellness care. These Terms and Conditions outline how we work together and set the expectations for our relationship. By using our website and services, you agree to these Terms.
Who We Are
Aevia Integrative Health provides wellness consultations, coaching, and health information focused on bioidentical hormone therapy, metabolic health, and gut health. Our practice is based in the United States and operates under the laws of the States of Pennsylvania and Maryland.
Scope of Services
Our services include hormone therapy management, nutritional guidance, and lifestyle coaching. While we provide medical expertise, our services are structured as a supportive partnership focused on long-term wellness. We do not provide acute care or emergency services.
Eligibility and Medical Suitability
To be eligible for our services, you must be a resident of a state where we are authorized to practice and be at least 18 years of age. Our providers will determine suitability for bioidentical hormone therapy based on medical history and diagnostic testing. We reserve the right to decline care if we believe our services are not clinically appropriate.
Consultations and Bookings
All consultations are scheduled by appointment only through our approved booking platform. You agree to provide accurate and complete information during the booking process. Your appointment time is reserved exclusively for you to ensure dedicated 1-on-1 focus.
Payments and Fees
Fees for consultations are clearly outlined at the time of booking and fees for labs and programs are outlined during the initial consultation. Payment is due at the time of service as specified in your care plan. We are a cash based clinic and do not accept insurance; nor can we provide care to Medicare recipients at this time.
Cancellations and Rescheduling
We value your time. If you need to cancel or reschedule, please provide at least 48 hours notice. Cancellations made with less than 48 hours notice may be subject to a fee. No-shows will be charged the full consultation fee to honor the provider's dedicated time.
No Medical Emergency Care
Aevia Integrative Health is not an emergency medical provider. If you are experiencing a medical emergency, including severe allergic reactions, chest pain, or shortness of breath, call 911 immediately or go to the nearest emergency room.
Telehealth Limitations
Virtual care has limitations, including the inability to perform in-person physical examinations. Technological issues may occasionally disrupt sessions. We will work with you to complete consultations via alternative means or reschedule if necessary.
Client Responsibilities
Your health is a collaborative effort. You agree to provide truthful and complete health history information. You are responsible for following the care plan developed with your provider and reporting any changes in your condition promptly.
No Guarantees of Results
Every body is unique. While we use science-backed therapies, we cannot guarantee specific outcomes. Success depends on various factors, including genetics, adherence to protocols, and individual hormonal response.
Governing Law
These Terms shall be governed by and construed in accordance with the laws of the United States and the States of Pennsylvania and Maryland, without regard to conflict of law principles.
